Required Permission Forms

For the 2019/20 school year, the District requires the following forms to be completed
using our SchoolCashOnline website (see below).

 School and District Code of Conduct
 Walking Field Trip Permission
 Media, Photo and Video Release Permission
 Digital Technology and Responsible Use Agreements
 Google Apps for Education Permission
 BC Fruit, Vegetable and Dairy Nutrition Program Permission
This form also requests sibling information for students who have siblings who attend a different SD40 school. We are collecting this information to help us build and manage family portal accounts in MyEd BC. Collecting sibling information will help prevent the creation of multiple accounts for families with children in multiple schools within SD40.

2019/2020 Student Emergency Release Form
The second form requests parents to provide at least two local emergency contacts who reside within walking distance to their child’s school who, if necessary, would be able to pick up their child in the event of an emergency.
